This is a brief description of the coverage provided by the Occupational Accident plan. The benefits described below are subject to certain limitations and exclusions. If any conflict should arise between the contents of this handout and the Policy or, if any point is not covered herein, the terms of the Policy shall govern in all cases.

Description of Coverage

This plan offers protection against certain injuries resulting from a covered accident while performing On-Demand Services1, subject to certain limitations.

Benefits Provided

If a participating independent contractor suffers a covered injury as the result of a covered accident, Intact may pay certain Benefit Amounts to the insured person or his or her designated beneficiary. If the accident results in more than one of these benefits, the amount payable will not exceed the Combined Single Limit.

1 On-Demand Service(s) means the period of time between the start and end of a shift, as recorded by OnCallforCare, LLC, during which an Insured Person is performing the contractual services as stated in the request from OnCallforCare, LLC. It does not include: 1) usual travel to, from and/or between the locations where the On-Demand Services are to be performed; or 2) Personal Deviations as defined in the Policy.
